This position is located in the Health Information Management Department at the Acoma-Canoncito-Laguna Service Unit in Acomita, New Mexico; work site location will also include rotation to Canoncito Health Center and Laguna Dental Clinic. The purpose of this position is to interpret, analyze and assign diagnostic and procedural codes. The coding function provides the primary source for data and information used in health care, promotes continuity of medical care, and ensures compliance with third party reimbursement policies, regulations and accreditation guidelines. The IHS is required by law to give absolute preference to qualified applicants who meet the Secretary of the Interior's definition of Indian for appointment to vacancies within the IHS in accordance with established IHS policy as outlined in the Indian Health Manual Part 7, Chapter 3. Medical Record Coding

  • Assigns and sequences a variety of codes including but not limited to International Classification of Disease (ICD-9-CM & ICD-10-CM), Current Procedural Terminology (CPT), Healthcare Common Procedure Coding System (HCPCS) codes based on the medical record analysis.
  • Assures the final diagnoses and procedures as documented by the provider are valid and complete.
  • Analyzes and abstracts information from the health record to identify secondary complications and co-morbid conditions to assure appropriate assignment.
  • Analyzes provider documentation to assure the appropriate Evaluation & Management (E&M;) levels are assigned using the correct CPT and/or HCPCS code.
  • Makes final determination that the medico-legal requirements of the health record is complete, accurate and reflects sufficient data to justify the diagnosis and medical necessity.
  • Abstracts and enters all data for coding, billing, statistical and governmental reporting measures required by regulatory agencies. Data will be entered in a timely manner with impeccable accuracy.
  • Coding, abstracting and data entry is submitted utilizing various computer information systems simultaneously. These systems include the Resource Patient Management System (RPMS), Patient Care Component (PCC) applications, the RPMS/Electronic Health Record (EHR), 3M Encoder, the Internet and various other applicable software applications.

Medical Record analysis

  • Perform comprehensive quantitative analysis by reviewing health records (paper and/or electronic) to assure the presence of all component parts of the ambulatory visit record; including correct identification and validation of each part by name, health record number, signatures and dates where required and the presence of all reports which appear to be indicated by the treatment rendered.
  • Perform comprehensive qualitative analysis by evaluating the ambulatory visit record for documentation consistency and adequacy. Ensure the final diagnosis accurately reflects the care and treatment rendered and documented within the health record.
  • Identifies inconsistencies, discrepancies and/or trends within the health record and discusses with the appropriate medical, nursing, or healthcare provider and recommends appropriate modifications to include medical necessity under the Correct Coding Initiative (CCI) with oversight by a tenured Coder.


  • Runs error reports to ensure data is transported to the data center. Make corrections to reports by validating visits within the database.
  • Maintain open lines of communication with the Business Office staff on reimbursement issues.
  • Maintain record confidentiality in accordance with federal regulations.

MINIMUM QUALIFICATIONS: Candidates for GS-6 must have had 52 weeks of specialized experience equivalent to at least the GS-5 level.

Specialized Experience: Specialized experience is experience in a hospital or clinic setting analyzing, interpreting and abstracting data from clinical documentation and/or electronic health record and assigning correct diagnostic and procedure codes. Candidates must have ability to communicate with clinical staff for specific coding and documentation issues.

Proficiency Requirements: In addition to meeting experience or education requirements, applicants for this position must show possession of Office Automation skills and must have typing proficiency of 40 words per minute.

Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).

Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee. IHS may offer newly-appointed Federal employees credit for their directly related previous non-federal experience or active duty uniformed military service. This credited service can be used in determining the rate at which annual leave is earned.
