- Job Summary
This position must be able to promote the physical and emotional well-being of the nursing facilities residents. Must understand and have experience with Resident Assessment Instrument (RAI) to gather information from the residents and their families during initial and periodic interviews. Will cover such areas as the patient's mood, behavior patterns, cognitive ability, and nutrition needs. This position will function in a lead capacity.
Information from these assessments helps nursing home caretakers formulate individualized care plans that include support from social services, dieticians, rehab specialists, and medical staff. This position will then implement and monitor these care plans to ensure their effectiveness. Must also make sure all strategies comply with Medicare requirements and ethical standards.
Requirescurrent RN Licensure in the State of Georgia. Valid driver's license and current automobile liability insurance required.
Requires two years MDS experience and two years experience as a nursing home supervisor/MDS Coordinator.Must have experience in MDS/Care plans and have knowledge and understandingof Nursing Home prospective payment system.Knowledge of Nursing Home reimbursement mechanisms including Medicare Parts A and B, Medicaid.Managed Care and other third party payers. Basic Computer skills.

Evaluation skills adequate to evaluate resident response to care and take appropriate action.
High level of interpersonal skills in order to effectively communicate with residents, families, visitors, teammembers, physicians and other personnel with whom daily interactions must take place.
Basic knowledge of and skills in management in order to interact effectively as a team member on a team withvarying levels of health care workers.
Problem solving skills sufficient to allow effective problem/conflict resolutions.
Demonstrates traits of initiative and dependability.
Ability to perform efficiently and effectively under stress.

Computer data entry skills and dexterity to accomplish data entry.
Sitting for periods of time in excess of 2 hours.
Walking for short periods.
Some moderate to heavy lifting at times, not to exceed 40 lbs. alone.
Sight to observe and assess residents/symptoms, read medical records and other facility relatedinformation.
Auditory to communicate verbally via telephone, with residents and to respond to emergencyalarms/signals.
Tactile to feel temperature variations and manipulate keyboard on computer. Olfactory to smell diseasespecific smells, safety hazards.
