  1. *Position Summary: *This position supports the mission of The Medical Center of Aurora/Centennial Medical Plaza of optimizing the quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. Responsible for intake/assessment/evaluation of behavioral health clients seeking Adult partial hospital/intensive outpatient/outpatient behavioral health services. Provides evaluation/assessment for determination of appropriateness of level of care utilizing the American Psychiatric Association Diagnostic and Statistical Manual of Mental Disorders, behavioral health, ASAM, and Interqual criteria. Facilitates transfer/referrals/admissions to intensive services as necessary pre-certification of medical care with insurance/managed care entities. Responsible for completion of biopsychosocial, integrated summary and remainder of Master treatment plan complying with 27- 10/ATS/ADAD/JCAHO/Medicare standards of care. Facilitates weekly staffing with regular review of clients in compliance with ATS/ADAD/JCAHO regulations. Responsible for coordination of care for dually diagnosed clients, facilitating didactic sessions, individual/group/marital/multi-family therapy groups, aftercare/discharge planning, transfers/referrals. Responsible for determining random urine/drug screen schedules on clients and appropriate follow through depending on results. Responsible for monitoring follow through with support group attendance/psychiatric visits/medications compliance/status of recovery. Facilitates discharge planning/referral services and completes discharge summary. Provides services to rehab/residential, partial hospitalization, intensive outpatient, and outpatient populations. Provides outpatient addiction consultations to 4B and med/surg on behavioral health issues and at risk population. When need arises, will perform other duties as assigned by supervisor.

Position Requirements:

*Licensure/Certification/Registration: *BLS-Healthcare provider, CPI certification within 6 months of hire and re-certification every 2 years thereafter, licensure in the State of Colorado (LCSW, LISW, LSW, LPC, LMFT, Psychologist), Colorado Addictions certification/licensure (CAC II or III) required. National/specialty certification (MAC, NADAC case-management, etc.) preferred.

Education: Masters degree in Social Work, Counseling, or related field from an accredited school required. Post Masters coursework in related field preferred.

Experience: Five to seven years of clinical/direct behavioral healthcare experience with minimum of two years in a primary therapist role/acute care setting required. Eight to twelve years of clinical/direct behavioral healthcare experience with minimum of five years in a primary therapist role/acute care setting preferred.

Special Qualifications: Must have excellent written, verbal and interpersonal communication skills and able to work with a highly diverse population. Must be knowledgeable of ADAD and JCAHO standards. Must be knowledgeable of most current American Psychiatric Association, Diagnostic and Statistical Manual of Mental disorders criteria as well as ASAM and Interqual criteria. Must be knowledgeable of behavioral health evaluation and consultation processes. Must have ability to perform multiple tasks simultaneously and work in high stress environment. Must be able to apply critical thinking and formulate diagnostic impressions based upon review of systems, history, symptoms and current presentation. Must be able to provide crisis intervention and utilize system resources/referrals.

Degree of supervision required: Involves general guidance and direction by the Director of Behavioral Health Services. Employee will be expected to perform most job duties independently and in accordance with established departmental and hospital policies and procedures.
