Greene Memorial Hospital/Soin Meidcal Center : Case Manager

The Case Manager is a registered professional nurse with education, knowledge and experience related to the specific unit assigned. The Case Manager’s role focuses on leadership in the multidisciplinary planning and coordination of the patient’s care across the healthcare continuum. The case manager organizes the sequence of patient care to attain maximum levels of effectiveness and efficiency to reach desired quality, clinical and cost outcomes. The case manager organizes multiple strategies to insure the care of all patients is managed with minimal overlapping roles and processes. The case manager participates in a team approach, composed of the physician, nurse, social service, and other direct and indirect providers and payers.



  • Registered Nurse with a license to practice in Ohio

  • Case Management CCM/ACM certificate required for all associate and diploma nurses within 3 years of hire, CCM, ACM preferred with bachelor or masters prepared Case Mgmt nurses.

  • BSN/BS from an accredited school preferred.


  • Five years clinical experience with two years experience in case management / one year utilization management experience (Interqual and Milliman) preferred

  • Works well in a multidisciplinary setting.

  • Works independently.

  • Good verbal and written communication skills to maintain good rapport with physicians, hospital administration, nurses and other health team members.

  • Ability to prioritize and work with multiple activities essential.

  • Computer skills related to data entry, management, retrieval, and presentation.

  • Familiarity with process improvement principles and their application preferred.

  • Knowledge of federal, state, JCAHO utilization and quality improvement regulations and their application needed.

  • Knowledge and experience with the MIDAS and MS Word required; Prefer experience with MS excel programs.

  • Ability to accept responsibility, analyzes problems, evaluate reports, make decisions and recommendations required.

  • Ability to demonstrate initiative and good judgment in a self-directed role.

  • Ability to use and to interpret variance and statistical data.

  • Must be flexible and able to manage job functions despite shifting priorities**
