The Cincinnati, OH VA Medical Center is recruiting for one RN Care Manager to demonstrate leadership and mastery of practice in delivering and improving efficient, comprehensive, and continuous patient-driven holistic care through active collaboration, communication and coordination of resources. She/he works in partnership with an assigned panel of patients and Patient Aligned Care Team (PACT) for effective disease and chronic care management. In PACT, the patient is the center of the care team that includes family members, caregivers, and health care professionals - a primary care provider, nurse care manager, clinical associate, and administrative clerk. The RN in this position must demonstrate leadership, experience, and creative approaches to management of complex client care.

The VAMC is currently seeking 1 (One) RN for the Care Manager of the Clermont CBOC. This position is within Primary Care and will report directly to the Manager of the CBOC's.

DUTIES: The RN Care Manager is directly supervised by the Primary Care Chief Nurse or Nurse Manager assigned to the area in which he/she works. Utilizing the nursing process and evidenced-based practice, the RN Care Manager collaborates with and leads his/her assigned PACT Teamlet in the development and implementation of the patient driven, holistic care plan for the veteran. She/he works with members of the expanded health care team and consultants as the patient's goals dictate. The RN Care Manager provides education, management and monitoring of chronic and acute disease processes and chronic pain; makes assignments and provides direction to the other members of the nursing staff. The RN Care Manager functioning as a preceptor assumes accountability and responsibility for the clinical development of assigned orientees. The RN Care Manager functioning as a charge nurse assumes accountability and responsibility for the care delivery provided the Primary Care patients assigned to the clinic, by the assigned staff, for an assigned tour of duty. The duties of this position include, but are not limited to:
Serves as a preceptor and mentor to colleagues and students, enhancing the professional growth, personal development and practice of others.
Demonstrates leadership by active involvement in area-based councils, committees, and task forces.
Administers medications and procedures per established policies, protocols and guidelines.
Demonstrates and maintains full competency in the use, pre-cleaning and/or reprocessing of reusable medical equipment. Ensures that the appropriate Standard Operating Procedure (SOP) and manufacturer's instructions are followed for any step in the use of RME (pre-cleaning, reprocessing, disinfection, sterilization, maintenance) in which she/he is involved.
Develops the plan for health education and assures the patient and his/her family receives the information needed to focus on self-management, prevention, and wellness, based on patient goals.
Collaborates with services internal and external to the VA to facilitate care transition in order to effectively meet the patient's needs.
TOUR OF DUTY: Full time, primarily Monday through Friday day shift, with occasional evenings and weekend requirements.
Travel: Ability to travel between all six CBOCs and the Cincinnati VA Medical Center.

- Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
- The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
1 Year of Medical/Surgical nursing experience is strongly preferred
BLS/ACLS REQUIREMENTS UPON SELECTION : Per VHA Directive 1177, current certification in BLS issued from the American Heart Association or Military Training Network is required for all staff providing direct patient care, and police. Additionally, current ACLS is required for patient care staff that work in specified or high risk areas (SICU/ICU/CCU/MICU/PACU/ED/and all procedure suites using moderate sedation, etc.). If you are selected for a position which requires BLS/ACLS certification, you will be required to have proof of certification (issued from the American Heart Association or Military Training Network) prior to being boarded for an appointment with the VA. (Failure to provide such certification can result in delays in the hiring process).

The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
- Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
- Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
- Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
- Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
- Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree in nursing or related field and approximately 2-3 year's of nursing practice/experience.

PHYSICAL REQUIREMENTS: This position regularly interacts with patients and therefore requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. The position requires walking, standing, stooping, sitting, bending, pulling and pushing. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations. This position requires a valid driver's license as the incumbent will travel between all CBOCs and to the Cincinnati VA Medical Center.
