Location: 2239 E. 55th St. Cleveland, OHShift: 7:00am - 3:30pm
Assist individuals referred from CCBDD and Medicaid funding sources to meet their basic living skill goals. Shape personal and social behaviors of consumers in a pleasant, therapeutic environment. Prepare and manage all individual case files and write detailed and accurate case notes on activities for each assigned consumer. Write monthly reports and conduct quarterly reviews regarding consumer’s progress. Assist the Program Supervisor with developing and implementing long and short-term individual support plans based on the consumer’s needs and goals. Attend annual and/or quarterly IP meetings as requested. Responsible for keeping the support administrator informed of the consumer’s progress and acts as a liaison to the referring support administrator, family, guardians and other individuals actively involved in the consumer’s life. Supervise lunch and break activities, aid in using the restroom, feeding, and other activities as necessary. Assist with all self help skills, including personal care. Recommend and implement program improvements and continue professional development through workshops and seminars.
Required Skills and Experience
Bachelor’s degree (B.A.) or 1 year of experience; or an equivalent combination of the two. GED or high school diploma is required. Previous experience with the MR/DD population is required. Experience with Microsoft Word and Outlook preferred. Strong communication skills both verbal and written are required.
