Didi Hirsch Mental Health Services Care Coordinator B/L in Culver City, California


Scope of Position

Case Managers provide comprehensive case management and counseling services to adults with a history of mental illness, in an outpatient and field-based setting to Spanish speaking clients. The ideal candidate will function with, and maintain a "whatever it takes attitude”.

Essential Function

  • Conducts/facilitates individual and group counseling and provides routine case management including assessment, intake, education, crisis intervention, and exit interviews for adults or adolescents with a history of mental illness and/or substance abuse.

  • Interfaces with such agencies as the Department of Mental Health, Criminal Justice Services, Public Health and other related community agencies and resources to give and receive information relative to clients.

  • Maintains documentation of all counseling sessions, substance abuse services /all other services, and ensures compliance with agency policies/procedures, contracts, and other regulatory agencies.

  • Supervises program participants with medication compliance, and drug screening as needed.

  • Provides crisis intervention, and facilitates conflict resolution among the program participants.

  • Prepares education materials for staff training and presentations.

  • Chaperons group sponsored trips and social functions; may drive seven-passenger van to transport program participants, escorts/transports individuals to appointments outside facility.

  • Develops and maintains counselor skills through seminars, workshops, or other forms of training/education.

  • Actively participates in all team and staff meetings, community meetings and supervision/consultation.


Required Qualifications

  • Bachelors Degree in related field preferred.

  • 1-2 years experience working with individuals with a history of mental illness in a social service/mental health environment.

  • Must be able to provide case mangement services in both English and Spanish (Read, Write, and Speak)

  • Knowledge of case management, crisis intervention techniques, community resources, and the ability to match clients with a history of mental illness to community support resources.

  • Proven comprehension of the recovery model desired.

  • The ability to provide services in a language other than English preferred.

  • Knowledge of program HIPAA, DMH, policies and documentation guidelines, licensing, and other state/federal regulations relating to service programs for adults with a history of mental illness.

  • Demonstrated advocacy skills.

  • Strong written and oral communication and PC skills.

  • Current California drivers license and a driving record acceptable to the Agency's insurance carrier.
