Social Worker III, Full Time

Position Summary:This position supports the mission of Presbyterian/St Luke’s Medical Center of optimizing the quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. The BMT Social Worker III is responsible for assessing the psychological, social, cultural and/or financial needs that impact the high-risk patient’s health and recovery. Has the ability to utilize social services, counseling and clinical knowledge in the assessment and treatment of these needs. Responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical and psychosocial approach through the continuum of care and balances individually identified patients and family needs with the efficacious and cost effective utilization of resources. Through concurrent case management, patients will be assessed to determine appropriateness of admission, continued hospitalization, as well as appropriate level of care. Discharge planning will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization. Patient care will be provided in the outpatient clinic and inpatient settings Knowledge of payer source implications on discharge plan, resources and placement options to maximize patients’ successful disposition. Assess and intervene with potential patients that are either uninsured or underinsured prior to scheduling a consultation to the Colorado Blood Cancer Institute (CBCI), and to notify the CBCI team as to how the insurance status impacts both treatment and treatment options. Provides evidence-based clinical intervention for a variety of emotional concerns including anxiety, depression, relationship concerns, etc. Functions as a part of a multidisciplinary team to bring patients successfully through the transplant process.

A. Licensure/Certification/Registration: Licensed Clinical Social Worker (LCSW) in the state of Colorado. Specialty certification in oncology preferred. BLS required within one month of hire (must be obtained prior to providing any direct patient care).

B. Education: Master of Social Work (MSW) from an accredited school of social work required.

C. Experience: 3-5 years recent clinical social work experience in a medical setting. 5-7 years recent clinical social work experience in a medical setting preferred.

D. Special Qualifications: Must have excellent written, verbal and interpersonal communication skills and able to work with highly diverse population. Must be knowledgeable of State and TJC standards. Must be knowledgeable of Interqual criteria. Must have ability to perform multiple tasks simultaneously and work in high stress environment. Must be able to apply critical thinking and formulate diagnostic impressions based upon review of systems, history, symptoms and current presentation. Must be able to provide crisis intervention and utilize system resources/referrals.

Degree of supervision required: Involves general guidance and direction by the Department Supervisor. Employee will be expected to perform all job duties independently and in accordance with established departmental and hospital policies and procedures.

Ages of Patients Served: This position requires knowledge of growth and developmental needs of patients and families. Personnel in this position must recognize and respond to the behaviors of patients and families in each age category. The employee must be able to demonstrate the special skills required to care for patients and their families according to standards. (The skills and knowledge needed to provide such care may be gained through education, training or experience.)
