POSITION SUMMARY: Provides primary care to Rehab Unit patients acting as an assistant to physicians in the inpatient rehabilitation unit following established standards and practices. As a Rehab Nurse Practitioner/Physician Assistant, he/she is involved in education of patients, families, physicians and staff.
SPECIFIC DUTIES: General: 1. Order Non-narcotic Pharmacological agents. 2. Order and interpret diagnostic studies. 3. Provide patient, family and staff education. 4. Promote core measures compliance. 5. Participate in education for the purpose of continuous quality improvement. Inpatient: 1. Accompany physicians on patient rounds. 2. Oversee appropriateness criteria for patient bed placement. 3. Transcribe and implement physician orders 4. Complete standard pre-procedure forms and review patient information. 5. Assist with discharge assessment and planning. 6. Dictate discharge summaries and conduct post-discharge follow-up Diagnostic / Procedures: 1. Assist with H & P 2. Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures needed. 3. Performs a complete physical exam and records findings. Collaborates with physicians in managing acute and long-term medical needs of patients. 4. Orders appropriate laboratory and diagnostic procedures. 5. Maintains and review patient records, charts, and other pertinent information. Post tests and examination results and communicates with patient's primary 6. Interview patients, write orders and facilitate discharge process 7. Collect patient information to approve continued stay in rehab 8. Participates in professional development activities and maintains professional affiliations. 9. Other duties as assigned.
EDUCATION REQUIRED AND/OR PREFERRED: (list required and preferred separately) · Graduation from an accredited school of nursing required; nurse practitioner program; physician's assistant program. · Successful completion of an accredited Nurse Practitioner or Physician Assistant program required. · Experience in acute care and management of Rehab patients preferred
LICENSURE/CERTIFICATION: (list required and preferred separately) · Current Advanced Nurse Practitioner or Physician Assistant License required. · Current certification in CPR and Advanced Cardiac Life Support required.
EXPERIENCE REQUIRED OR PREFERRED: (list required and preferred separately) · Minimum of five years clinical Rehab experience in nurse practitioner / PA preferred · Critical care experience preferred. · Genuine interest in hospitalist doctoring preferred. · Genuine interest in Rehab Medicine preferred. · Genuine interest in geriatric medicine preferred. · Bilingual in the Spanish Language preferred. · Effective leadership, teaching , research and communication skills required · Ability to demonstrate complex clinical skills within the framework of the advanced practice nurse or physician's assistant. · Advanced physical assessment and critical thinking skills required. · Knowledge of medical equipment and instruments required. · Ability to assess medical conditions and interpret findings required. · Ability to collaborate with multidisciplinary team required. · FIM Credentialed within one year of hire required.
