The incumbent will be assigned to the Office of Nursing and Patient Care Services, reporting directly to the Home Based Primary Care (HBPC) Program Manager. The incumbent will function as program coordinator and is administratively responsible for the Medical Foster Home (MFH) Program. The coordinator will manage the daily operation of the program and provide direct patient care services in the program area.

Major Duties:

  • Responsibility of daily operation of the MFH program;
  • Developing policies and procedures for program operation;
  • Preparing reports and statistics for facility, VISN, and national use of the program;
  • Manage and control the use of data related to the program regarding day to day functioning and program evaluation

Basic Requirements:

  • Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website ( to verify if that social work degree meets the accreditation standards for a master of social work.
  • Licensure. Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.
  • English Language Proficiency. Applicants appointed to direct patient care positions must be proficient in [both] spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
  • Physical Standards: VA Directive and Handbook 5019, Occupational Health Services states that all applicants must be physically and mentally able to perform efficiently the duties of the position. Applicants for this position must pass a pre-employment medical examination.

Preferred Experience: Capable of practicing at the advanced level and has specialized knowledge of practice typically related to a particular diagnosis or population for at least 2 years.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

Grade Determinations: Social worker program coordinator must be licensed or certified at the advanced practice level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. In addition, the candidate must demonstrate the following Knowledge, Skills and Abilities:

  • In-depth knowledge of the program coordinated, and demonstrated knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
  • Knowledge and skill in management/administration, which includes supervision, consultation, negotiation, and monitoring.
  • Ability to supervise multidisciplinary staff assigned to the program.
  • Ability to organize work, set priorities, meet multiple deadlines, and evaluate assigned program area(s).
  • Ability to provide training, orientation, consultation and guidance within clinical specialization of practice.
