Walworth County Certified Nursing Assistant-2nd Shift, Part-Time (CNA) in Elkhorn, Wisconsin

This position is responsible for assisting in the personal cares of the residents of Lakeland Health Care Center (LHCC) based on the interdisciplinary plan of care under the direction and supervision of the Unit Supervisor.

SCHEDULE

Days: Set schedule based on position

Work Hours: 2:45pm - 11:15pm or 4:00pm - 8:00pm

Hours Per Shift: 4 or8

Shifts Per Pay Period: Based on position Shift Premium: $0.25/hr received for any hours worked from 2:45pm - 7:30am

Essential Duties and Responsibilities:

This job description reflects management's assignment of essential functions; it does not prescribe or restrict the tasks that may be assigned.

Reporting changes in resident's physical and/or mental status to the Unit Supervisor immediately.

Providing personal hygiene including oral care, bathing, hair shampooing, shaving, skin care, nail care, and hand washing as well as assisting with toileting including bowel and bladder retraining and perineal care.

Assisting/performing resident mobility, transfers, ambulation, position changes and range of motion.

Serving residents meals and assisting residents during meals.

Serving meals for accurate portions according to menu spreadsheets.

Interpreting modified diet menus and preparing accordingly.

Recording food temperatures prior to service.

Cleaning and sanitizing food preparation utensils, dishes, equipment and resident serving/dining areas.

Monitoring refrigerators for proper labeling, dating, storing, rotating and use of foods.

Ordering food supplies from the main kitchen.

Preparing, cooking or baking foods according to posted breakfast menu with assistance from the cooks.

Employing appropriate assistive equipment such as gait belts, mechanical lifts and correct body mechanics when lifting, positioning and/or transferring residents and/or their belongings.

Documenting cares given such as temperature, pulse, respiration, weight, intake/output and ambulation on flow sheets, target behavior forms, nutritional intake sheets and required electronic charting.

Assisting residents in maintaining the cleanliness and neatness of residents' rooms including making beds and disposing of soiled linens and clothing.

Reviewing Certified Nursing Assistant (CNA) Summary Cards for assigned residents daily and discussing suggested changes with Unit Supervisors as needed as well as assisting with pre-staffing information collection.

Using standard precaution for infection control as well as using isolation techniques, including the use of a particulate mask.

Performing simple treatments as assigned by the Unit Supervisor within the limits of ability, training and guidelines of the Wisconsin Administrative Code Chapter HFS129.

Participating in at least twelve (12) hours each year of inservice education programs to increase knowledge and skills as requested to maintain certification on the Wisconsin State Registry as well as attending mandatory inservices.

Delivering care in compliance with the Resident Bill of Rights by maintaining confidential resident care information and respecting residents' rights and privacy.

Assisting with orientation of new CNAs on the unit as necessary.

Assisting CNA students with clinical experience.

Communicating problems or concerns with team members to assure issues affecting resident care outcomes or general operations of the facility are handled efficiently.

Responding to emergency situations per LHCC Emergency Response Plan.

Responding to resident emergencies such as bleeding, falls, seizures and choking, per policies and procedures.

Assisting with post mortem care.

Assisting with the admission of new residents.

Labeling of residents' personal items.

Participating in Quality Improvement (QI) activities.

Assisting residents to get ready for medical appointments.

Assisting residents to safely board the transportation vans.Monitoring positioning and safety of residents during transport.

Assisting residents at the appointment site.Upon return to the facility, assisting resident back to their room and helping with ADL's, as needed.

Education and/or Experience

High school diploma or equivalent competencies, and zero to six months of relevant prior experience.Knowledge of medical terminology and basic arithmetic skills is required.

Knowledge, Skills & Abilities

Ability to perform manual tasks associated with personal care of elderly.

Knowledge and understanding of Resident Bill of Rights.

Ability to work independently and as part of a team.

Good oral and written communication skills.

Certificates, Licenses, Registrations and Other Special Requirements

Wisconsin Nursing Assistant Certification, and on State Registry for Nursing Attendants.

Criminal background check free of convictions of abuse of individuals or misappropriation of property while in a formal caretaking setting.

All staff of Lakeland Health Care Center may be called upon to respond to emergency situations assisting residents in their physical positioning.

If you are not available to work on the date this position starts, you will not be awarded the position.

All employees may be required to work overtime as determined by the department head.
