Walworth County Public Health Nurse (BSN) (Health & Human Services) in Elkhorn, Wisconsin
This position is responsible for applying public health principles, interventions, and essential services including educating, counseling, preventing disease and promoting wellness and safety to individuals, families and visitors across the life span.The public health nurse collaborates with internal and external partners such as WIC, Children's Services, Behavioral Health, schools, private providers, and other public and government agencies on community planning, projects and programs.

Days: Schedule to be determined based on needs of department. Schedule to be based on 40 hours in a two-week pay period
Work Hours: Normally between the hours of 8:00 a.m. - 5:00 p.m. Some evenings may be required.

Essential Duties and Responsibilities:

Provide health assessment and educational services to individuals and groups using public health interventions based on best practices for the identified public health population.
Provide immunization services in accordance with Federal and State immunization protocols.
Provide working knowledge of epidemiologic investigations, surveillance and control of communicable disease as mandated by HFS Chapter 145 and case management of referral and follow-up with consumers and providers as applicable.
Provide nursing assessments, education, intervention, case management and referral services for at risk families, including prenatal, postpartum, infant and children populations.Assess for cultural, environmental, sociological and psychological factors that may influence family health status and safe environment.Refer as identified.
Provide Well Child/Health Check assessments and screening services to Public Health consumers, birth through 18 years.
Provide Chronic Disease assessment and screening services primarily aimed at cancer, diabetes, and cardiovascular risk reduction.
Participate in emergency preparedness initiatives and exercises.Complete introductory series of National Incident Management System (NIMS) including Incident Command System (ICS) for public health and safety emergencies.
Respond to Public Health emergencies, as needed, including outside regular office hours.
Prepare and maintain Public Health Department records and reports in accordance with agency policies and State data requirements.
Collaborate with other county departments, area service providers, schools and other civic organizations to identify health needs of the community, creating and facilitating programs that promote a broad continuum of primary, secondary and tertiary prevention of illness.
Demonstrate a commitment to county safety and risk management efforts.

Education and/or Experience
Bachelor's degree in nursing from an accredited program required. Experience in public health, home health, health education, or independent living preferred. Previous experience in case management preferred. CPR certification preferred. Proven experience in ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals as well as ability to compute rate, ratio and percent and to draw and interpret bar graphs.

Provides specialized information and/or recommendations to others regarding an area of expertise. Ability to work with multi-ethnic, multi-cultural populations.
Certificates, Licenses, Registrations
Valid Wisconsin driver's license
Proof of minimum liability insurance coverage
Wisconsin Registered Nurse through WI Department of Safety and Professional Services.
