Diabetes Instructor – Deran Koligian Ambulatory Care Center at Community Regional Medical Center


The Diabetes Instructor serves as one of the coordinating hubs of the Diabetes Care Center Program. You will keep the primary physician informed about the patient’s status and provide ongoing instructions to the patients. You will also manage patient and physician phone calls, complex clinic schedules for patients undergoing diabetes education, and assists with the creation of patient education materials. Your role is essential to excellent patient care and the Diabetes Care Center program success.

This position provides diabetes education to patients with diabetes by assisting with the organization of required health care services.

In this role you will work closely with a team of physicians and nurses to perform quality patient assessments and interventions and formulate treatment according to protocols. You will evaluate the patients’ nutritional needs and formulate diet plans.

Provide continuing education for health care professionals. Actively participates in staff training and professional organizations. Networks with other health care professionals to improve interagency cooperation and increase knowledge base

Respond to patient inquiries and makes appropriate recommendations and treatment adjustments

Develop patient education materials and presents diabetes education programs

Participate in continuing education and training on an ongoing basis to retain and enhance competency of practice

Serves as a liaison between the Diabetes Patient Education Advisory Committee and the center's administration

Minimum Requirements:

Current California Registered Nurse license or Registered Dietitian license. Certified Diabetes Educator Certificate or must obtain within two years from date of hire or transfer into job. Current BLS Certification

Preferred: Bilingual Spanish Speaking
