We are currently seeking an experienced Case Management/Social Services Manager! The Manager is accountable for the oversight of the department operations including care coordination, and discharge planning functions. In collaboration with the medical and clinical staff, the delivery of excellent care is an organization-wide goal. The Case Management Manager assists the director in planning, organizing, and directing activities designed to address the clinical and psychosocial needs of patients and families. Additionally, this role will maintain and monitor service area targets vs. actual performance to meet operational goals. The Manager will coach and mentor staff through the application of corporate structure and patient focused care principals. This position will provide a rewarding sense of accomplishment as the department is a critical component of caring for a highly complex patient population. The ideal candidate will collaborate with both facility and service line leaders to ensure quality continuity of care throughout the continuum.
To qualify for this exciting leadership opportunity, you will need to have a LCSW or MSW with 10 years of management experience. A successful candidate should have demonstrated expertise in case management and leadership ability.
