The VA Central California Health Care System is recruiting for a full-time Physician in Geriatrics Extended Care Service. Candidate must have caring attitude and desire to do Geriatric medicine. Flexibility is essential. Willing to work collaboratively with a well developed interdisciplinary team across a continuum of care including a 54-bed community living center, Home Based Primary Care PACT, Geri-Med Primary Care PACT. Major Duties and Responsibilities include: Geriatric evaluation and management as well as skilled care, rehab, palliative care and hospice; Management of complex medical conditions, pain management issues, co-morbid mental health illness, psychosocial conditions; Longitudinal care for geriatric population with complex syndromes and complex clinical needs; Collaborates with ACOS Geriatric Extended Care in Performance Improvement activities; Other duties as assigned
Work Schedule: Full-time, Monday through Friday, 8:00 am to 4:30 pm. Physicians are on-call 24/7. On-call duties in the Community Living Center will be shared with others.

In order to qualify for this position you must meet the following:
Must be a U.S. Citizen.
Must be proficient in written and spoken English.
Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine.
Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
Completed a first year residency, or its equivalent, approved by the Secretary of Veteran Affairs for the year in which it was completed.
- Board Certified in Internal Medicine and Certification in Geriatric Medicine.
