Description The Cardiac Service Line RN is a professional registered nurse with advanced education, knowledge, and experience in patient, family, and team education and care coordination as it relates to patients within the Cardiac Service Line for KMCS. They demonstrate the ability for clinical nursing practice as it relates to patients within the Cardiac Service Line. The Cardiac Service Line RN provides disease education and support for the Structural Heart Program and participates in the daily outpatient clinic operations – assisting patients transitioning between inpatient and outpatient settings. This role focuses on leadership in the multidisciplinary planning and coordination of patient/family education and care. The Cardiac Service Line RN also participates in a multidisciplinary team approach for coordination of the patient’s healthcare continuum. They target disease specific and community-focused areas that are identified within the cardiac care continuum, but may also identify other barriers or issues that could impact patient outcomes and facilitate solutions. The Cardiac Service Line RN will assist in coordinating “Morbidity and Mortality” (M&M;) Committees for: Cardiac Surgery Nursing, Cardiac Surgery and the combined Surgery/Cardiac Cath Lab. The Cardiac Service Line RN will assist with data collection, entry and reporting of TVT registry and quality cardiac metrics and outcomes. Other duties as assigned.
Requirements Registered Nurse with current license to practice in the State of Ohio BSN Required, MSN preferred
3-5 years of clinical experience in a cardiac specialty Knowledge of computer and electronic health record systems. Strong verbal and written communication skills.
Experience in protocol/care path development, implementation and monitoring preferred. Expertise in healthcare clinical quality improvement preferred.
