This position is located in the Primary Care Health Care Group at the VA Long Beach Healthcare System in Long Beach, California. The incumbent selected for this position will be under the direct supervision of the Nurse Supervisor of the clinic to which assigned. He/she utilizes the nursing process and evidence-based practice to collaborate with members of the Patient Aligned Care Team (PACT) and larger team (family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient-driven holistic care plan for life. The RN collaborates with patients to assess and identify needs, issues, care goals, and resources in critical factors for achieving desired outcomes for discharge, post hospital recovery, and health maintenance/wellness. The RN is responsible for the direct care of the patient . The RN is primarily accountable for assessing, planning, implementing, and evaluating direct and indirect nursing care, and identifying outcomes for patients in the outpatient clinical setting. He/She is responsible for coordinating patient care delivery to ensure quality care according to established standards of care/practice, policy and procedure. The specific patient population to whom direct care is provided includes adults with different conditions. Other key responsibilities include: Collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patient's needs. Promoting patient and family health education with a focus on self-management, prevention, and wellness, based on patient's goals. Providing both team and disease case management for a panel of patients assigned as applicable. Utilizing multiple communication modalities to meet the needs of the patient including telephone, secure messenger and office visits and documenting interactions
Responding to MyHealtheVet/Secure Messaging requests from veterans/family members within 24 administrative hours of receipt
Promoting an environment that embraces new knowledge, innovations, and improvements. Demonstrating leadership by serving as an advocate for patients, team leader to colleagues.
Communicating regularly with nurse managers, physicians, nursing supervisors, charge nurses, transfer coordinators, allied healthcare staff, healthcare technicians, Pharmacy, and the business office Traveling to other Primary Care Outpatient Clinics may be required
DESIGNATED DRUG-TESTING POSITION: Applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. - Registration Current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia. The RN must maintain a current, full active and unrestricted registration to continue employment with the VA
- Educational requirements can be met as follows:
(1) Graduate of a school of professional nursing approved by the appropriate State agency, and accredited by one of the following accrediting bodies at the time the program was completed: a) The National League for Nursing Accrediting Commission (NLNAC); or b) The Commission on Collegiate Nursing Education (CCNE). OR
(2) The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. OR
(3) In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration, will meet the requirement of graduation from an approved school of professional nursing.
The following requirements are preferred for this position:
- At a minimum, applicants should possess experience as an RN that is determined to be sustained and consistently at or above an acceptable level of quality. This may include experience as a Graduate Nurse Technician (GNT) provided that candidate was utilized as a professional nurse and passed the State licensing (board) examination on the first attempt, and experience as Nurse Technician Pending Graduation provided the candidate possessed an active, current registration to practice nursing in a State and was utilized as a professional nurse. Professional nursing experience should be documented on the Application for Employment Nurse and Nurse Anesthetists (VA Form 10-2850a).
- One year (or more) working experience as a registered nurse.
- Two (2) - Three (3) years of working experience in a medical-surgical setting.
