The Transplant/VAD Coord. (RN) delivers professional nursing care and education in the preoperative and postoperative settings to patients undergoing transplant procedures.
Evaluates, monitors and educates transplant patients prior to transplant procedures.
Provides post-operative nursing care; monitors for and identifies complications such as infection and organ rejection. Provides discharge education to patient and family.
May participate in transplant education and research initiatives.
May serve on committees for the purpose of developing and implementing the organization's transplant policies and procedures.
Coordinates the care of transplant and/or VAD patients throughout the continuum of care beginning with the initial referral through post -transplant/implant long term follow up care.
Serves as the primary point person for the multidisciplinary transplant and/or VAD team, through a call rotation, for patient calls, hospital staff calls and transplant offers.
Assures medical record collection and thorough medical record reviews are completed for all patients referred into the system to determine a patient’s candidacy for implant and/or transplant.
Implements patient plan of care in collaboration with the multidisciplinary transplant and/or VAD team and effectively communicates any patient concerns.

BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date.
Licensed Registered Nurse credentialed from the Tennessee Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date.
- Associate's degree in Nursing required; Bachelor's degree in Nursing preferred
- Four years minimum experience required with Associate's degree in Nursing; Two years minimum experience required with Bachelor's degree in Nursing.
