Local Summary: Officially established in October 2006, the Orlando VA Medical Center is one of the Top 100 Companies in Central Florida for working families. The Orlando VA Medical Center includes the following facilities; a 120-bed Community Living Center and a 60-bed Residential Rehabilitation Program (located at the Lake Nona Campus), Outpatient Clinics in Orlando (Lake Baldwin), Viera and Daytona Beach and four Community Based Outpatient Clinics located in Clermont, Kissimmee, Leesburg, and Orange City. Our new medical center is currently open for many outpatient services and will continue to open more services, including inpatient beds and an emergency department, throughout 2016. The new 65-acre medical campus has 134-inpatient beds and provides acute care, complex specialty care, advanced diagnostic services, a large multispecialty outpatient clinic, and administrative and support services.
Position Summary: The Orlando VA Medical Center is seeking for a Registered Nurse (RN)/Case Manager to facilitate and coordinate between members of the Medical Center and non-VA providers in the local community for outpatient services that cannot be provided by the VA. He/she will process consults utilizing clinical judgment and VA guidelines to determine if services can be provided inter-facility or need to be referred for non-VA care. This Case Manager functions independently, demonstrating decision making capacity and is responsible for contacting non-VA providers for authorization of medical care at VA expense. He/she will collaborate with the veteran and interdisciplinary staff members to maximize healthcare responses, quality and cost effective outcomes. This RN provides follow-up and ensures continuity of care during the time the veteran is receiving non-VA services. A variety of administrative functions are necessary for smooth management, including but not limited to: correspondence/ telephone calls, faxing, tracking tools. The incumbent reports to the Nurse Manager, Care Management.
Work Schedule: Normal work days/hours will be Monday-Friday, 8:00 AM to 4:30 PM, with work day/hours flexibility required as administrative and/or clinical demands require.

a. US Citizenship and English Language Proficiency required.
b. Graduate of a school of professional nursing approved by the appropriate State agency and accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
c. Current, full, active and unrestricted licensure as a registered nurse (RN) in the State of Florida required.
d. Bachelor of Science Degree in Nursing (BSN) is highly desired, with Master's Degree in Nursing or a related field preferred. National certification in Case Management is also highly desirable.
e. Recent/current case management experience (within the past 5 years) preferred.
f. Demonstrated ability to work independently and exercise good judgment with minimal supervision.
g. Demonstrated dependability for duty and flexibility in work assignments and hours in previous assignments.
GRADE DETERMINATIONS: Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows: Practice: practice, ethics and resource utilization; Professional Development: performance, education/career development; Collaboration; collaboration, collegiality; Scientific Inquiry: quality of care, research.
