Position Description :
The Diabetes Practice Nurse functions to facilitate patient flow through the practice to meet patient care needs in collaboration with a multidisciplinary team of providers, including physicians, nurses, dietitians, and medical assistants. This nurse is responsible and accountable for the supervision and training of the medical assistants/LPN, for the organization of education materials and referral resources for patients, for teaching all topics in the diabetes education program, and for maintaining JCAHO/OSHA requirements. Furthermore, this nurse is responsible and accountable for professional growth and for maintaining certification as a Certified Diabetes Educator (CDE).
Minimum Requirements :
Graduate of an NLN-approved school of nursing
Current RN licensure in the commonwealth of PA.
Certification as a Certified Diabetes Educator (CDE) preferred, or required within two years from the time of employment
2-3 years experience within the field of diabetes
