The Community Health Nurse position is located at Four Corners Regional Health Center, Red Mesa, Arizona; Dzilth-Na-O-Dith-Hle Health Center, Bloomfield, New Mexico and Northern Navajo Medical Center, Shiprock, New Mexico. The Community Health Nurse plans, coordinates and evaluates a comprehensive community health nursing program and assists the community in planning and evaluating a local health care delivery system. This position reports to the Supervisory Community Health Nurse.

- Conduct epidemiological and community surveys, field investigations and research studies to appraise health needs.
- Assess and diagnose needs and set up a plan of action; participate with the interdisciplinary team; work with Tribal, State and County agencies in planning health related activities.
- Plan and provide a comprehensive nursing service to individuals, families and communities; provide skilled nursing services to individuals and families in homes, schools and/or outreach clinics.
- Provide patient teaching and assist them in understanding, accepting and adjusting to the implications and responsibilities of medical care, diagnosis and treatment.

- Travel is required to make home visits.

To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. *BASIC REQUIREMENT(S): Education: *A minimum of a baccalaureate or higher degree in nursing from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed.
Registration: Applicants must have an active, current registration as a professional nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
Minimum Qualifications: In addition to the above you must meet the following minimum qualifications for the grade level you wish to be considered for:
GS-07: One (1) year of specialized experience equivalent to the GS-05 level.
GS-09: Two (2) full years of progressively higher level graduate education OR a master's degree or equivalent degree OR One (1) year of specialized experience equivalent to at least the GS-07 level.
GS-11: Completion of all requirement for a doctoral degree (Ph.D. or equivalent) or 3 full years of progressively higher level graduate education OR 1 year of specialized experience equivalent to at least the GS-09 level.
Graduate education must have been in nursing with a concentration in a field of nursing (e.g., teaching, a clinical specialty, research, administration, etc.) or in a closely related non-nursing field directly applicable to the requirements of the position to be filled.
Specialized Experience: Experience must have equipped the applicant with the particular knowledge, skills and ability to perform successfully the duties of the position and that is typically in or related to the work of the position. For example, experience as a community health nurse who provides primary health care and generalized public health nursing services in a clinic, home, hospital, school and/or assists the community in planning and evaluating a local health care delivery system.
The selected individual is required to maintain nursing licensure requirements. If nursing licensure is not maintained during employment, the employee will be subject to adverse actions, up to and including removal from Federal service.
Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).
