The St. Louis VAMC is a full-service health care facility providing inpatient and ambulatory care in medicine, surgery, psychiatry, neurology, and rehabilitation, as well as over 65 subspecialty areas. It is a two-division facility that serves veterans and their families in east central Missouri and southwestern Illinois.

The John Cochran Division has all of the medical center's operative surgical capabilities, the ambulatory care unit, and a six-story Clinical Addition that includes surgical facilities, intensive care units, outpatient psychiatry clinics, and expanded laboratory.

The Jefferson Barracks Division provides psychiatric treatment, regional spinal cord injury treatment, a nursing home care unit, geriatric health care, rehabilitation services, and a rehabilitation domiciliary program for homeless veterans.

The St. Louis VA Medical Center is associated with Community Based Outpatient Clinics (CBOCs) in Belleville, IL; O'Fallon, MO (St. Charles County); and St. Louis, MO (North St. Louis County); and Washington, MO.

The Nurse Practitioner in Extended Care and Rehabilitation Service duties include consultation services, outpatient appointments, potential for home visits, as well as primary provider responsibilities for Veterans on the Community Living Center (CLC) admitted under the Hospice treating specialty. Additionally, duties include a primary care panel as well as primary provider responsibilities for Veterans on the CLC admitted under Palliative Care. The Nurse Practitioner is an Advanced Practice Generalist who has specific education and expertise in end of life care, specifically palliative and hospice care. The Nurse Practitioner maintains knowledge of current practice, acts as a role model and mentor to other APRNs, acts as a client advocate, and participates in professional activities at the local and state level.

Duties include:

  • Investigate health problems or health needs by gathering and assessing historical information

  • Perform a physical examination

  • Diagnose health problems based on interpretation of the patient's history, physical exam findings and diagnostic test results

  • Develop a treatment plan tailored to the patient's needs, which includes health education, pharmacologic and non-pharmacologic treatment

  • Prescribe medications as appropriate

  • Evaluate the results of the treatment plan, modifying the plan as needed to achieve the treatment goals

  • Make referrals for specialty evaluation or treatment

  • Write admission and discharge summaries

  • Identify hospice candidates, place hospice consults, oversee implementation and assure peaceful death for the resident

  • Facilitate and promote the delivery of quality care through comprehensive pain and symptom assessment, informed decisions regarding end-of-life and timely reassessment of the effectiveness of interventions including pharmacologic and non-pharmacologic intervention and consultations with other pain specialists

This position reports to the Associate Chief Nurse, Extended Care and Rehabilitation Service and works in partnership with their collaborating physician for medical issues. The VA maintains an electronic medical record and all staff are expected to gain proficiency using this medium.

  • U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

  • Full unrestricted Licensure, Certification, or Registration required

  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency andaccredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.

  • The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)


  • Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience.
  • Nurse III - MSN or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.

Preferred Experience:

  1. MSN with NP program completion required.

  2. Certification as an adult or family practice NP by a national certifying body required.

  3. Previous Geriatric and/or Hospice/Palliative Care/end of life experience preferred.

  4. Two to three years clinical practice experience in the role as a Nurse Practitioner preferred.

The Physical requirements of this position include the following tasks which may be performed intermittently: lifting of up to 50 lbs. (3 hours), sitting (2 hours), standing (4 hours), walking (8 hours), climbing (1 hour), kneeling (3 hours), bending/stooping (3 hours), twisting (2 hours), pushing/pulling (6 hours), simple grasping (8 hours), fine manipulation (8 hours), reaching above the shoulder (4 hours), and operating machinery (8 hours).
