The Spinal Cord Injury Center Licensed Vocational Nurse (LVN) is a position within the Patient Care & Nursing Services' at VA San Diego Healthcare System located in La Jolla, CA. The LVN is accountable for the quality of nursing care he or she provides to patients and utilizing the nursing process, assuming responsibility for planning, implementing, directing, and evaluating nursing care for assigned patients. The scope of functions and the variety of tasks depend on the area of assignment. The LVN performs services requiring technical and manual skills and obtains basic patient data, i.e., vital signs, weight, medications taken, input and output; assists in managing bowel and bladder care; participates in planning; executes interventions in accordance with the care plan or treatment plan; and contributes to evaluation of individualized interventions related to the care plan or treatment plan. The duties require provision of both administrative and clinical support for the Spinal Cord Injury Center and under the supervision of a licensed Registered Nurse (RN). The primary purpose of the Spinal Cord Injury Center LVN position is to provide a full range of services and nursing care to patients who present for or request assistance with a variety of mental, physical and/or behavior problems. The LVN provides care to patients with the goal of promotion and/or maintenance of health and/or prevention of illness and injury based upon the nursing process which includes performing services requiring technical, manual and basic assessment (data collection) skills. The scope of functions and the variety of tasks depend on the area of assignment. The LVN also contributes to a safe, accessible, efficient and effective patient centered environment of care through active participation in Privacy, Customer Service, and Planetree-in-services, as well as infection control, emergency preparation and preparedness plans. Fosters/supports a learning environment by participation in the orientation of new employees, providing feedback as appropriate and seeking opportunities for self-improvement. The LVN's primary commitment is to be patient. (ANA Code of Ethics). The LVN is able to recognize and report potential safety hazards and life threatening emergencies and respond appropriately. As an active member of multidisciplinary care team this nurse understands the importance of reporting and recording, in the patient's computerized medical record, the care provided, pertinent interaction(s) and communication(s), with patients, family members, professional and supportive personnel.

Work Schedule:Full Time, 40 hours per week Not to Exceed 13 months; Weekends/Weekdays/Holidays/Overtime may be needed. Specific work hours will be discussed in interview.

Basic Requirements:

  • U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

  • Education. Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant.

  • Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory, or Commonwealth.

  • Physical Standards

  • Proficient in both written and spoken English.

Preferred Experience: At least one (1) year of full time employment as a SCI nurse; Minimum of one (1) year of LVN, SCI Acute Care w/ventilator and Rehabilitation Nursing Experience; Ability to perform complex tasks correctly; and the ability to communicate effectively and resolve conflict constructively.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

Grade Determinations: Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment.

GS-5: Completion of at least one (1) year of qualifying experience at the GS-4 level or equivalent. The applicant must demonstrate the following competencies: Demonstrated knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. Works with the RN and/or MD to appropriately orient/train less experienced LPNs and/or Nursing Assistants /Health Technicians in relation to acceptable standards of practice in promoting optimal patient care delivery. Provides effective education to patients and/or family members in relation to common disease processes, medication, and /or prescribed treatment regimes.

  1. Demonstrated ability to serve as a responsible member of the nursing team and interact in an appropriate manner with patients, family members, professional and other supportive personnel involved in the delivery of patient care, incorporating acceptable, established customer service standards into practice.

  2. Knowledge and skill sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications as permitted by approved local facility policies and procedures. Observation and documentation will include patient's response to medication administered and the reporting of any noted change in patient's condition to RN or MD.

  3. Knowledge and ability to recognize the need for and to institute emergency measures when indicated, promptly seek the assistance of the RN or MD, and assist in resuscitation procedures in cardiac and/or pulmonary arrest.

  4. Recognizes and appropriately responds to breakage/malfunction or loss of equipment, safety hazards, and supply deficiences, promptly reporting to appropriate personnel for corrective action.
