Ascension Medical Group (Ministry Medical Group) Anticoagulation/CoumadinTherapy is looking for a skilled and passionate Registered Nurse to join their Stevens Point team on a Casual basis ! This clinic is located on the campus of Saint Michael’s Hospital.
As a Registered Nurse in the Anticoagulation Clinic, you will:
Coordinate patient care through collaboration with the patient/family and managing medical provider for anticoagulation therapy. Perform job duties utilizing medical provider approved Anticoagulation protocols and communicate with medical provider as needed to provide quality patient care. Enter initial patient data into the Electronic Health record. This includes indications for warfarin therapy, length of therapy, baseline labs, co-morbid conditions that may influence therapy, current tablet strength of warfarin, and any previous dosing history. Nurse enters medications, allergies, and updates the problem list as needed. Provide initial patient education related to Anticoagulants to patient and family at an age appropriate level. Assess ongoing learning needs for education and reinforcement of patient education. Perform fingerstick INR test in the Anticoagulation Clinic and enter accurate fingerstick PT/INR results into the appropriate documentation system. Retrieve INR results, interpret INR results and make dose adjustments if necessary utilizing established protocols. Communicate with patient, family pharmacy or homecare agency regarding test results and any dose adjustments necessary. Assess patient compliance with medication regimen, changes in medications, diet, health status, bleeding or falls, illnesses or recent hospitalizations as appropriate with each INR test. When Lovenox® (enoxaparin) bridging is indicated, the Anticoagulation RN will order labs as required pre/post bridging, educate patient of self-injection technique, and provide written day by day instructions pre and post procedure. Nurse will communicate with ordering medical provider for Anticoagulation therapy and procedurals regarding plan of care. Nurse obtains prior authorization for medication if required by patients insurance.
This is a casual position, meaning there is no guarantee of a set number of hours per week or benefits offered. Hours for this position will be between 7:45AM and 5:15PM. We are looking at a minimum of working three (3) shifts per month and attend required trainings and meetings. Qualifications: Graduate of an approved school of nursing. Two years of nursing experience; Clinic or hospital setting preferred. Licensed in the State of Wisconsin to practice as a Registered Nurse
Skills Required: Ability to function independently in a clinic setting; excellent interpersonal and assessment skills, observant of confidentiality at all times. Effectively communicates with patients, families, physicians, staff members and other health-care professionals.
