Unit 4K is a 17 bed Pulmonary/Respiratory medical-surgical unit with services provided for General Medicine Patients, Airborne Isolation and Surgical overflow. Patients served are medicine patients ranging from mid 20's to early 90's receiving treatment for a variety of medical diagnosis such as diabetes, COPD, GI bleed, CVA, Cellulites, Pneumonia, Congestive Heart Failure, Peripheral Vascular Disease, ETOH withdrawal, Schizophrenia, Coronary Artery Disease.

Central Texas Veterans Health Care System (CTVHCS) is one of the largest integrated health care systems in the Department of Veterans Affairs and includes facilities in Temple, Waco, and Austin, TX. Community-Based Outpatient Clinics are located in Brownwood, Cedar Park, Palestine, and Bryan-College State. NO STATE TAXES!!!

*Tour of Duty: To be determined by service. Primary tour of duty is days (7:30 am - 4:00pm), however, rotation to non-administrative tours to include evenings, nights, and weekends may be indicated on occasion to impact desired outcomes. *

DUTIES: Administratively responsible to the ACNS for Inpatient/Critical Care and is responsible and accountable for the coordination of care for a culturally diverse veteran population. The Clinical Nurse Leader (CNL) possesses the knowledge and skills to effectively apply all aspects of the nursing process and care management principles within a collaborative, interdisciplinary practice setting, which will then enable effective implementation of health promotion and prevention practices, management of acute and chronic illnesses, and attainment of appropriate lengths of stay and most effective level of care on the assigned patient care unit(s). Will demonstrate knowledge and skills necessary to provide cost effective care. This includes understanding changes associated with aging and principles of growth and development relevant to the adult and geriatric population. The CNL meets all mandatory requirements for assigned area and will perform activities that reflect the educational, experiential and competency requirements.

The (CNL) develops a high level of clinical practice, leadership and skills to improve/coordinate patient care for an adult population. Practice settings may include Critical Care, Primary Care, Acute Care, Mental Health and/or Geriatrics. The practice of each CNL is based on knowledge, experience, and research, expected to impact patient outcomes in the assigned area and for the facility. Executes position responsibilities that demonstrate leadership, experience, and creative approaches in the management of complex patient care.

  • The Clinical Nurse Leader will design, initiate and evaluate individualized systems of care using multiple data sources that include:

  • Holistic assessments

  • Problem identification

  • Application of acute and chronic care/management models, theories and concepts

  • Communication with interdisciplinary team members

  • Performance metrics and regulatory standards

  • The Clinical Nurse Leader assumes accountability for patient care outcomes related to nursing care provisions.

  • The Clinical Nurse Leader demonstrates ability to perform, teach, delegate and mentor nursing and other staff.

  • The Clinical Nurse Leader functions as a leader and partner in the interdisciplinary team.

  • The Clinical Nurse Leader demonstrates the ability to perform, teach, delegate and supervise nursing procedures with safety and competence.

Preferred Experience/Education: MSN prepared and one or more years of experience as a Clinical Nurse Leader. Will consider BSN/MSN with the expectation of obtaining CNL certification within 18 months.

  • U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.

  • The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)

  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

*Grade Determinations: *The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:

  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.

  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.

  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; ORa Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.

  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; ORa Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.

  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree in nursing or related field and approximately 2-3 year's of nursing practice/experience.

Preferred Experience:

  1. BSN and approximately 2-3 years of experience; or

  2. MSN and approximately 1-2 years of experience; Masters in a field related to nursing and a BSN and approximately 1-2 years of experience; or

  3. Doctoral degree in nursing, or in a field related to nursing and no experience.

Physical Requirements:This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
