Summary: The Care Assure Program, Nurse Navigator, also known as Care Assure Supervisor is responsible for the programs and resources that support the navigation of patients through complex healthcare environments for the optimization of care and outcomes. The Care Assure Supervisor facilitates patient care coordination through patient coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex disease, and streamline the intervention process. The Supervisor guides patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. S/he serves as a point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services.


Graduate of an accredited School of Nursing and current licensure as a Registered Nurse in the state of California required. Bachelor’s degree required, Masters preferred. Current BCLS card required. Knowledge and application of pertinent computer programs and imaging software (i.e., MUSE, PACS, etc.). Hospital Fire & Safety Card within 60 days of hire.

Experience/ Ability/Skills:

Minimum 3 - 5 years relevant specialty area experience strongly preferred, i.e. Cardiovascular, Case Management, Orthopedic, Neurology, Spine, Oncology. The incumbent of this position must demonstrate strong verbal communication skills, computer, problem solving and analytics skills, and effective organizational skills with an inter-disciplinary focus. Demonstrate knowledge in the application of pertinent computer programs and imaging software (I.E. MUSE, PACS, etc.); MS Office, hospital based computing systems (i.e. Meditech).
